Access to the RPA patient portal is by invitation only. If you have already received an invitation to our new FollowMyHealth portal and created your login please Click Here to login to your child’s account.

 

Fall 2015:

We are now sending out invitations for our new FollowMyHealth Patient Portal.

Some features available on the patient portal at this time include: Appointment Requests, Medication Refill Requests, and General Messaging. In addition, you will be able to view your child’s immunization records and vitals.  More features will become available on the portal during future roll-out phases.  We encourage you to become familiar with and to begin using the patient portal.

If you would like to request an invitation for a child 17 years of age or younger, please complete the Patient Portal Invitation Request Form and return it by fax, postal mail or drop off.  For HIPAA purposes, we do not accept forms via e-mail.  A Patient Portal Invitation Request Form may be downloaded by clicking here.

For patients 18 years and older, please complete the Patient Portal Invitation Request Form for Patients 18 Years & Up and return it by fax, postal mail, or drop off.

If you have any technical issues involving our portal, please call our office and leave a message in our portal support mailbox. Someone will return your call within 24-72 business hours.
